Cure disease cast on the host expels it outright, no save. Attacking it directly with anything else simply hurts a sick person.
    Saves
    Quick 1 / Tough 2 / Mind 1

The sickness that arrives with a purpose behind it. A guecubu does not have a body of its own and does not want one - it rides an animal, or a person, and wastes them from the inside while using them to reach the next. The creature attacking you is a victim. It has been ill for a fortnight, it is dying, and it will die whether or not you kill it, and the guecubu will simply move.

    Special Attack

Waste: necrotic, melee 5 ft, +4 to hit, 2d6+3, delivered with the host's own hands. Spread: 20 ft, on the host's death or when it chooses, save Con Tough, fail save roll 2d6+2 necrotic and the guecubu moves into that creature as its new host, cooldown 2 rounds.

    Special Defense

Rides a Victim: all damage dealt goes to the host, which is dying anyway. There is no separate creature on the field. Driven Out: rites of cleansing, smoke, or a healer's work performed on the host expel it without killing anyone. Cornered: with no living creature within 20 ft when its host dies, it is destroyed, which makes an empty room the whole tactical problem.
