    A Futakuchi-onna appears human until her hair parts to reveal a second mouth at the back of the skull. The hidden mouth speaks private hungers, stretches the hair like grasping limbs, and consumes everything denied by the visible face. It may arise from starvation, cruelty, or concealed appetite. The two mouths do not always want the same future.

    Special Attack

    Hungry Hair: slashing, 15 ft, +6 to hit, 3d8+5. A hit holds the target and pulls it 10 ft toward the hidden mouth.

    Special Defense

    Two Voices, One Body: an appeal directed separately to each mouth forces a Mind save. On a failure, the mouths argue and the creature loses its next attack. Cutting bound hair releases every held target.
